We are at 16311 Ventura Blvd, Suite 1296, on the Encino stretch of Ventura Boulevard between Hayvenhurst and Balboa. The building has valet parking and we validate it — in full for first appointments and treatment appointments, partially for routine cleanings — so there is no need to circle the block looking for a meter.
Plan on about an hour for a first visit. New-patient forms can be emailed to you beforehand or filled in when you arrive, whichever you would rather do. Bring your insurance card if you have one, a list of anything you take, and any recent x-rays another office may be holding.
